    Abstract Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) and Long COVID are complex, multisystemic conditions that pose ongoing challenges to healthcare professionals. Emerging research suggests that ME/CFS and Long COVID exhibit overlapping metabolic symptoms, indicating possible shared metabolic dysfunctions. This study aims to systematically explore these shared metabolic disturbances and their potential treatments. Utilizing our novel metabolic modeling method, GPMM, we identified the key metabolic irregularities in patients with ME/CFS and Long COVID, notably the downregulation of the alanine and aspartate metabolism pathway, and the arginine and proline metabolism pathway. Genome-wide knockout analyses indicated that supplementation with aspartate (ASP) or asparagine (ASN) could potentially ameliorate these metabolic deficiencies. Further metabolic assessments in Long COVID patients highlighted the significant downregulation of ASP in both blood and muscle, supporting our predictions. Consequently, we propose that the combination of l-ornithine and l-aspartate (LOLA) offers a promising approach to alleviate metabolic symptoms in both ME/CFS and Long COVID patients. This study not only elucidates the shared metabolic pathways in ME/CFS and Long COVID but also positions LOLA as a viable candidate for future clinical trials.

    Institute news Erin Howden Associate Professor Erin Howden is one of 12 researchers nationally to be awarded a Medical Research Future Fund (MRFF) grant aimed at improving the lives of people with long Covid. The grants are the first investments from a $50 million commitment from the Federal Government to generate better evidence on the effective management of long COVID in the Australian community. Fatigue, dizziness, tachycardia and brain fog are among the most commonly described symptoms of long Covid/post-acute sequalae of Covid-19 (PASC). These symptoms are also commonly experienced by individuals affected by postural orthostatic tachycardia syndrome (POTS) and myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). There’s been considerable interest about the potential overlap between PASC, POTS and ME/CFS, but there’s been no detailed investigation of the shared pathophysiology of these conditions. A/Prof Howden’s $800,000 grant will allow her to combine MRI scans with direct recordings of sympathetic nerve activity to understand how the brain is regulating blood pressure during stressors like orthostatic challenge and exercise in patients with long Covid, POTS and ME/CFS. By understanding the mechanisms contributing to long COVID, she hopes to be able to direct treatment strategies that alleviate or resolve symptoms to restore the quality of life for people with long Covid.

Tucker June 11, 2024 Add to Email Alerts 0 7 A new broadly inclusive definition of long COVID from the National Academies of Sciences, Engineering, and Medicine (NASEM) has been developed with the aim of improving consistency, documentation, and treatment for both adults and children. According to the 2024 NASEM definition of long COVID issued on June 11, 2024, "Long COVID is an infection-associated chronic condition that occurs after SARS-CoV-2 infection and is present for at least 3 months as a continuous, relapsing and remitting, or progressive disease state that affects one or more organ systems." People with long COVID may present with one or more of a long list of symptoms, such as shortness of breath, rapid heartbeat, extreme fatigue, post-exertional malaise, or sleep disturbance and with single or multiple diagnosable conditions, including interstitial lung disease, arrhythmias, postural orthostatic tachycardia syndrome (POTS), myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), diabetes, or autoimmune disorders. The condition can exacerbate preexisting health conditions or present as new ones.
